Near Miss Reporting


    At NPI, safety is everyone's responsibility.
    A "near miss" is an unplanned event that did not cause harm — but had the potential to.
    Reporting it helps us prevent future incidents.

    Please report if you noticed:

    • Unsafe acts or unsafe conditions

    • Equipment failure or malfunction

    • Spills, leaks, or near-spills

    • Slips, trips, or falls (without injury)

    • Any situation that could have caused harm
